Description

Job Title: Nurse Practitioner (In-Home Assessments)

Location: New York

Compensation: $125,000 - $170,000 + Travel Stipend

Sign-On Bonus: $15,000 (Upfront with claw-back within the year)

Position Overview: We are seeking an enthusiastic Nurse Practitioner willing to travel statewide to join our care team in New York. In this role, you will play a crucial part in providing comprehensive healthcare services to patients throughout their healthcare journey, offering a unique opportunity to make a significant impact on patient outcomes and promote continuity of care.

Key Responsibilities:

  • Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
  • Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
  • Manage and monitor chronic health conditions, ensuring optimal disease control and patient education.
  • Prescribe medications, adjust dosages, and educate patients on proper administration and potential side effects.
  • Promote preventive healthcare measures and provide patient education on disease management, healthy lifestyles, and self-care practices.
  • Collaborate closely with healthcare professionals to ensure coordinated care and seamless transitions for patients.
  • Maintain accurate electronic health records (EHR) to track patient progress, treatment plans, and interventions.
  • Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
  • Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
  • Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
  • Participate in after-hours on-call and conduct longitudinal care and IHA visits as needed.
  • Weekly travel throughout the state. Travel arrangements will be based on patient needs and business needs. Drive to client and/or patient sites within a designated area.
  • Other duties as assigned based on business needs.

Qualifications:

  • Active, unrestricted Nurse Practitioner licensure in New York.
  • Certification as a Family or Adult-Gerontology Nurse Practitioner.
  • Minimum 3600 hours of practice experience or 3 years of clinical experience in a primary or longitudinal care setting.
  • Active driver's license with a clean record.
  • Current Basic Life Support Certification.
  • Active DEA license.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and patient education abilities.
  • Proficiency in electronic health record (EHR) systems.

Preferred Qualifications:

  • Experience with HEDIS/STAR measures.
  • Home-based visit experience.
  • HCC documentation experience.

Schedule:

  • Full-time (Five days a week, Monday to Friday, 8 am to 5 pm, with a minimum of 25 visits a week)
  • Part-time (Four days a week, with a minimum of 7 visits a day)
